- Academic Certificates: Including FSLC, WASSCE, SSCE, GCE, NECO, and NABTEB with at least five credits in English, Mathematics, Chemistry, Physics, and Biology. Note: Only a maximum of two sittings are accepted, except for NABTEB which cannot be combined with other results.
- Professional Testimonial: From the last professional school attended.
- Proof of Identity: Birth Certificate or Age Declaration
- Practicing License: Current and valid.
- Change of Name Certificate: If applicable.
